Social Worker – Adult Services (Qualified)
£33 per hour
Contract: Locum | Full Time - Monday to Friday

Please note: All candidates must be able to drive and have access to their own vehicle, as well as a Social Work Qualification and 3 years permanent employment experience.
Job Overview

Lancashire County Council is seeking an experienced and motivated Qualified Adult Social Worker to join the team based at Fylde Family Support Centre, supporting adults across the Fylde & Wyre area.
This role sits within the Single Point of Access (SPA), with a strong focus on hospital triage and discharge planning, ensuring safe and timely transitions from hospital to the community.
Key Responsibilities

• Triage and assessment within the SPA, supporting hospital discharge pathways
• Apply eligibility criteria in line with the Care Act 2014
• Complete Continuing Health Care (CHC) Assessments and Reviews
• Undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005
• Work collaboratively with health professionals and partner agencies to support safe discharges
• Maintain accurate, timely case records in line with statutory and local authority requirements

Essential Requirements

• Qualified Social Worker with current Social Work England registration
• Previous experience or strong working knowledge of hospital discharge pathways
• Proven ability to carry out Care Act assessments, CHC assessments, and Mental Capacity assessments
• Ability to work effectively in a fast-paced, multi-disciplinary environment
• Full UK driving licence and access to your own vehicle
