Candidate Responsibilities:
- Conduct thorough assessments of adult clients' needs and circumstances
- Develop, implement, and review care plans tailored to individual requirements
- Support service users in accessing relevant services and resources
- Collaborate effectively with multidisciplinary teams, external agencies, and families
- Maintain accurate records and ensure compliance with all regulatory requirements
- Promote independence, safety, and wellbeing for vulnerable adults
- Participate in team meetings, case reviews, and professional development activities
- Relevant social work qualification and registration (e.g., HCPC registration)
- Proven experience in adult social work or mental health settings
- Strong assessment, communication, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of current social work legislation and policies
- Competitive hourly pay rate of 27.61 LTD per hour inclusive of holiday pay or 21.70 PAYE exclusive of holiday pay
- Contract length of 11 months, with potential for extension
