Adult Social Worker - Care & Support Team

Location: Powys County Council
Rate: £34.72 per hour Umbrella (£26.55 per hour PAYE)
Contract: 3-month temporary contract with a view to extension
Hours: Full-time, Monday to Friday

Service Care Solutions are recruiting for an experienced Adult Social Worker to join the Care & Support Team at Powys County Council.

This is an excellent opportunity to join a supportive team, working with adults who require assessment, care planning, and ongoing support to achieve positive outcomes.

Key Responsibilities

• Undertake assessments and reviews in line with relevant legislation.
• Manage a caseload of adults with varying levels of complexity.
• Develop and implement person-centred care and support plans.
• Work collaboratively with service users, carers, families, and partner agencies.
• Promote safeguarding and wellbeing while ensuring effective risk management.
• Maintain accurate and up-to-date case records.

Requirements

• Qualified Social Worker with a recognised Social Work qualification.
• Registered with Social Care Wales.
• Previous experience working within Adult Social Care.
• Strong assessment, communication, and case management skills.
• Ability to work effectively within a multi-disciplinary environment.
