Key Responsibilities
- Manage a caseload of adults requiring safeguarding and mental health interventions.
- Undertake assessments, reviews and support planning in line with statutory duties.
- Complete safeguarding enquiries and risk assessments.
- Work collaboratively with health professionals, partner agencies and service providers.
- Support adults to achieve positive outcomes whilst promoting independence and wellbeing.
- Maintain accurate and timely case recordings and reports.
- Qualified Social Worker.
- Registered with Social Care Wales.
- Experience working within Adult Social Care.
- Knowledge of safeguarding procedures and relevant legislation.
- Experience working with individuals with mental health needs.
- Strong assessment, communication and partnership-working skills.
- Ability to manage a varied and complex caseload.
- Full-time office attendance required during induction.
- Following induction, a hybrid working arrangement is available, with 3 to 4 days per week in the office and 1 to 2 days working remotely, subject to suitability and service requirements.
