Main Responsibilities
- Support individuals to meet their basic needs, including eating, drinking, and personal care, while maintaining dignity at all times.
- Follow individual eating and drinking guidelines and report any concerns promptly to the Speech and Language Therapy Coordinator.
- Promote physical wellbeing by following Occupational Therapy and Physiotherapy guidance and assisting with therapeutic movement sessions.
- Develop and adapt your communication style in line with personalised communication plans, using creative communication methods.
- Contribute to individual support plans and advocate for people’s choices and preferences.
- Maintain accurate written updates, including daily activity records and risk assessments.
- Support individuals with safe travel to and from the service, ensuring mobility aids and equipment are secure.
- Maintain the cleanliness and safety of the community hub environment.
- Follow all safeguarding, health and safety, and equality policies and procedures.
- Ability to communicate empathetically and effectively with people with PMLD.
- Calm, patient, and adaptable approach to providing support.
- Consistent and person-centred working style in line with support plans.
- Ability to motivate and celebrate each individual’s achievements.
- Competence in using a range of verbal and non-verbal communication techniques.
- Ability to write short, clear updates (handwritten or via email).
- Team player with a constructive and supportive attitude.
- Commitment to completing the Care Certificate within 12 weeks of starting.
- Awareness of safe working practices and the safety of others.
- Understanding of the diverse needs of local residents.
- Flexibility to work across different services as required.
- Experience supporting individuals with PMLD or similar complex needs.
- Experience using IT systems within a care environment.
