Job Title: Care Coordinator – Integrated Learning Disabilities Service
Department: Adult Social Care
Reports to: Team Manager
Location: Council / Community Based (Hybrid where applicable)
Contract: Initially 3 months, Full time 35 hours per week
Salary: £20 UMB per hour ( £16 PAYE)
Working with Enfield Council, We are looking for a committed and compassionate Care Coordinator to join their Integrated Learning Disabilities Service. This is a rewarding opportunity to support adults with learning disabilities and their carers by delivering high-quality assessments, care management, and coordinated support that promotes independence, wellbeing, and person-centred outcomes.
You will be part of a supportive multidisciplinary environment, working alongside social workers, health professionals, and partner agencies to ensure people receive the right support at the right time.

About the Role As a Care Coordinator, you will:
- Undertake initial needs assessments within your specialist area
- Develop, manage, monitor and review care and support plans
- Work closely with service users, carers, and partner agencies
- Coordinate services and placements using available community and independent resources
- Ensure care packages remain appropriate, effective, and value for money
- Maintain accurate and timely records using council IT and case management systems
- Work within financial regulations, safeguarding duties, and council policies
- Participate in supervision and performance reviews with your line manager
- Experience or strong ability in assessment and care planning
- Understanding of the care management process
- Ability to work in partnership with service users, carers, and professionals
- Knowledge of safeguarding processes and responsibilities
- Confidence working within financial and policy frameworks
- Strong written and verbal communication skills
- Professional, respectful, and person-centred approach
- Ability to take responsibility, collaborate, and problem-solve
- Experience in integrated or multidisciplinary teams
- Experience supporting people with learning disabilities and/or mental health needs
- Ability to use alternative communication approaches
