Are you passionate about supporting adults to live independently and thrive in their communities? We’re looking for a Case Assessor to join our Adult Social Care team and make a real difference in people’s lives.
About the Role As a Case Assessor, you’ll undertake less complex assessments and reviews, providing advice, information, and access to services. You’ll work closely with social work colleagues, providers, carers, and external agencies to ensure seamless, high-quality support for individuals in line with the Care Act (2014).
You’ll empower adults to make informed choices, conduct capacity assessments, arrange advocacy where needed, and maintain accurate records in line with statutory and departmental requirements.

Key Responsibilities
- Undertake needs-led assessments and reviews, designing outcome-based care packages.
- Work collaboratively across teams, providers, and agencies to deliver comprehensive support.
- Ensure all services are delivered in compliance with the Care Act (2014) and Mental Capacity Act (2005).
- Identify and report vulnerable adults at risk of abuse, following safeguarding policies.
- Advocate for service users and support them to make informed choices.
- Maintain up-to-date knowledge of social care developments and ensure accurate record-keeping.
- GCSEs/NVQ2/Level 2 Diploma or equivalent, including Maths and English – Essential
- Strong knowledge of the Care Act (2014) and Equalities Act (2010).
- Understanding of social care and health services and their interconnections.
- Excellent communication, negotiation, and consultation skills.
- Ability to form positive working relationships with service users.
- Proven experience in assessments, inter-agency working, problem-solving, and team collaboration.
- Competent IT skills and accurate record-keeping.
