Role Overview As a Community Care Officer, you will be the first point of contact for individuals requiring social care support. You will undertake strengths-based assessments and interventions in line with the Care Act 2014 and the Mental Capacity Act 2005, ensuring that people receive timely, person-centred support that promotes independence and well-being.

Key Responsibilities
- Respond to referrals from adults and carers who may require care and support
- Carry out assessments, support planning, and reviews in accordance with statutory requirements
- Apply the principles of the Care Act 2014 and Mental Capacity Act 2005 in daily practice
- Support individuals to access universal services, community resources, and informal networks
- Promote self-directed support, independence, and community inclusion
- Work creatively and collaboratively to achieve positive outcomes
- Maintain accurate records and case notes in line with professional and legal standards
- Participate in duty and rota systems, which may include some weekend work
- Work flexibly and proactively as part of a small, high-performing team
- Full UK driving licence and regular access to a vehicle for work purposes
- Previous experience working in social care (statutory or voluntary sector)
- Excellent interpersonal and engagement skills with a wide range of individuals
- Ability to assess risk, promote safeguarding, and apply legal frameworks appropriately
- Strong organisational skills and ability to manage a varied and dynamic caseload
- A commitment to inclusive practice and promoting independence in the community
- Flexibility to work occasional weekends, as required by the team rota
- Resilient and able to manage challenging situations with professionalism
- Imaginative and creative in problem-solving and support planning
- Reflective and committed to continuous improvement and learning
- A collaborative team player who contributes to a supportive working environment
- Open to smart and flexible working arrangements
- Understanding of direct payments and personal budgets
- Familiarity with digital case management systems
