Community Care Officer (Grade F) – Multiple Opportunities Available Nottingham City Council | Adult Social Care | 37 Hours Per Week | Initial 5-Month Agency Contract

Service Care Solutions are currently recruiting for multiple Community Care Officers (Grade F) to join Nottingham City Council's Adult Social Care Team. These are fantastic opportunities for experienced social care professionals who are passionate about making a real difference to the lives of vulnerable adults across the city.

Working within a supportive and collaborative team, you will play a key role in delivering high-quality assessments, care planning, case management and safeguarding interventions, ensuring adults receive the right support to promote independence, wellbeing and positive outcomes.

The Role As a Community Care Officer, you will manage a varied caseload, working with adults who have a range of complex needs. You will be responsible for assessing needs, coordinating services, managing risk and ensuring that individuals receive person-centred, outcome-focused support.

You will work independently under the guidance of Senior Practitioners and the Team Manager whilst collaborating with a wide range of professionals to deliver effective care and support.

Key Responsibilities
  • Complete strengths-based assessments for adults and carers.
  • Develop, implement and review person-centred care and support plans.
  • Manage an allocated caseload, ensuring timely reviews and interventions.
  • Undertake safeguarding work to protect vulnerable adults.
  • Assess and manage risk while promoting independence wherever possible.
  • Coordinate care packages and work closely with partner agencies.
  • Maintain accurate, evidence-based case recordings and reports.
  • Participate in duty rotas, responding to referrals and providing advice where required.
  • Attend multi-disciplinary meetings, supervision sessions and team meetings.
  • Build strong working relationships with health professionals, partner organisations and internal teams.
  • Support colleagues through advice, mentoring and collaborative working where appropriate.
  • Ensure all work complies with legislation, policy, data protection requirements and best practice standards.
About You To be successful in this role you will have:

  • Experience working within Adult Social Care.
  • Strong assessment and care planning skills.
  • Knowledge of safeguarding adults and relevant legislation.
  • Experience managing complex caseloads.
  • Excellent communication and report writing abilities.
  • The ability to prioritise workloads and work independently.
  • Experience working collaboratively within multi-disciplinary teams.
  • Confidence using electronic case management systems.
