We are seeking a compassionate and motivated Registered Nurse Associate (RNA) to join our community nursing team, delivering high-quality care to patients in their own homes. This role is ideal for a nurse who is passionate about person-centred care and supporting individuals to remain independent within their community.
You will work as part of a multidisciplinary team, carrying out planned home visits, supporting Registered Nurses, and contributing to the ongoing assessment and care of patients with a range of health needs.

You will work as part of a multidisciplinary team, carrying out planned home visits, supporting Registered Nurses, and contributing to the ongoing assessment and care of patients with a range of health needs.
- Deliver safe, effective, and compassionate nursing care during home visits
- Support patients with long-term conditions, post-discharge care, and wound management
- Undertake clinical tasks in line with your scope of practice and training
- Monitor, record, and report patient conditions and escalate concerns appropriately
- Maintain accurate and timely clinical documentation
- Work collaboratively with Registered Nurses, GPs, and other healthcare professionals
- Promote patient dignity, independence, and wellbeing
- Registered Nursing Associate (RNA) with current NMC registration
- Experience in community, primary care, or home visiting is desirable (but not essential)
- Strong communication and interpersonal skills
- Ability to work independently and manage a community-based caseload
- Compassionate, reliable, and committed to high standards of care
