About the Role Medway Council is seeking dedicated and experienced Education, Health and Care (EHC) Coordinators to join our SEND and Inclusion Service. This is a fantastic opportunity to make a real impact on the lives of children and young people with special educational needs and disabilities (SEND).
In this pivotal role, you’ll be responsible for coordinating and managing Education, Health and Care (EHC) Plans for children and young people aged 0–25, ensuring that each plan is outcome-focused, person-centred, and compliant with statutory requirements.
You’ll work collaboratively with families, schools, colleges, health and social care professionals to ensure that every young person has the right support in place to thrive and achieve their full potential.

Key Responsibilities
- Coordinate and manage the end-to-end process of EHC Plan development, maintenance, reviews, and amendments.
- Ensure all EHC Plans are compliant with the Children and Families Act 2014 and the SEND Code of Practice (2015).
- Collaborate effectively with parents, carers, schools, and multi-agency professionals to develop high-quality, outcome-focused plans.
- Provide guidance, support, and clear communication to families, ensuring a transparent and inclusive process.
- Maintain accurate records, track progress, and contribute to service improvement initiatives.
- Proven experience managing Education, Health and Care Plans (EHCPs), including pre- and post-14 transitions.
- Previous local authority experience in a SEND or EHC coordination role.
- Strong understanding of SEND legislation, statutory guidance, and best practice in multi-agency working.
- Excellent organisational and communication skills, with the ability to manage competing priorities effectively.
- Ability to work hybrid, attending the Gun Wharf Office and Medway Hubs at least three times per week.
