Service Care Solutions is recruiting experienced Care Coordinators to support a range of GP practices and Primary Care Networks (PCNs) across Manchester. Whether you're looking for full-time hours or flexible ad hoc shifts, we have immediate opportunities available with competitive pay and ongoing support.

Key Responsibilities

• Coordinate patient care and ensure timely access to appropriate healthcare services.
• Support patients with long-term conditions and complex health needs.
• Work closely with GPs, Practice Nurses, Social Prescribers, and the wider multidisciplinary team.
• Arrange referrals, follow-ups, and appointments with internal and external healthcare providers.
• Maintain accurate and up-to-date patient records using clinical systems.
• Signpost patients to community, voluntary, and statutory services where appropriate.
• Monitor patient pathways to ensure continuity of care.
• Provide excellent patient service while maintaining confidentiality and GDPR compliance.

Candidate Requirements

• Previous experience working as a Care Coordinator within a GP Practice, Primary Care Network (PCN), or Primary Care setting.
• Experience using EMIS Web (essential).
• Excellent communication and organisational skills.
• Ability to manage patient caseloads, referrals, and care pathways effectively.
• Confident working independently and as part of a multidisciplinary team.
• Strong understanding of patient confidentiality and data protection.
• Immediate availability is desirable.
