Extended Personal Advisor – Leaving Care Multiple Positions Available | Countywide

Are you passionate about supporting care leavers to build confidence, independence and a positive future?

We are currently recruiting for multiple Extended Personal Advisor positions within a Care Leaver Team, supporting older children in care and care leavers as they make the transition into adulthood and independent living.

This is a fantastic opportunity for an experienced social care professional who wants to make a genuine difference to the lives of young people.

The Role As an Extended Personal Advisor, you will hold a caseload of older children in care and care leavers, providing practical and emotional support as they prepare for and move into independence.

You will play a key role in:

  • Holding a caseload of older children in care and care leavers.
  • Overseeing and contributing to Pathway Planning.
  • Supporting young people to develop the skills, confidence and resilience needed for independent living.
  • Helping prepare looked-after children for adulthood and independence.
  • Providing practical support to care leavers as they navigate education, employment, accommodation and other areas of adult life.
  • Working alongside social workers, families, carers and multi-agency professionals.
  • Providing transport to children, young people and parents/carers where required.
  • Maintaining accurate records, reports and case documentation.
  • Assessing information and making sound decisions in relation to young people's needs.
  • Building positive relationships with young people and advocating for their best interests.
  • Working creatively and proactively to improve outcomes.
  • Attending supervision and contributing to your own professional development.
About You We are looking for someone who has relevant experience supporting care leavers or young people within a social care setting, alongside a genuine commitment to helping young people achieve positive outcomes.

You will ideally have:

  • A Level 3 childcare qualification such as NNEB, NVQ Level 3 or equivalent.
  • Experience working with care leavers and/or young people leaving care.
  • An understanding of the needs and challenges faced by young people transitioning into adulthood.
  • Knowledge of safeguarding, child development and relevant legislation relating to care leavers.
  • An understanding of the roles and responsibilities of statutory and voluntary agencies supporting young people.
  • Excellent communication and interpersonal skills.
  • Strong organisation and time-management skills.
  • The ability to maintain clear, accurate records and reports.
  • The ability to manage competing priorities and work to deadlines.
  • Confidence working collaboratively with professionals across multiple agencies.
  • Good IT skills, including email and Microsoft Word.
  • A creative, preventative and outcomes-focused approach.
Essential Requirements
  • Relevant experience working with care leavers/young people in a social care or similar setting.
  • Level 3 childcare qualification or equivalent.
  • A valid UK driving licence and access to a vehicle, as the role requires travel across the county. Reasonable adjustments will be considered where appropriate.
  • Willingness to work outside standard office hours when required, including occasional early mornings, evenings or weekends.
  • Successful completion of an Enhanced DBS check with the relevant barred list check.
Why This Role? This is much more than a case-management position. You will have the opportunity to become a consistent and trusted professional in the lives of young people who are preparing to take a significant step into adulthood.

If you have the experience, patience and determination to help care leavers thrive rather than simply survive, we'd love to hear from you.

Multiple positions are available, so if you're an experienced Personal Advisor or social care professional looking for your next opportunity, please get in touch.
