Family Help Lead Practitioner Children’s Services | Hybrid Working

We are seeking a dedicated Family Help Lead Practitioner to join a forward-thinking service focused on safeguarding children and helping families thrive. This is a fantastic opportunity for an experienced practitioner who is committed to strengths-based, trauma-informed practice and collaborative working across services.
In this role, you will play a key part in supporting families, coordinating multi-agency interventions, and ensuring children remain at the centre of all decision-making.
The Role As a Family Help Lead Practitioner, you will work directly with children, young people, and families to assess needs, coordinate support, and deliver evidence-based interventions that lead to sustainable, positive outcomes.
You will take a whole-family approach, working alongside partner agencies to ensure children are safeguarded and supported within safe, nurturing environments.
This role involves a mixture of community-based work, office working, and occasional home working, giving you the flexibility to manage your workload effectively while maintaining strong engagement with families and professionals.

Key Responsibilities
- Undertake holistic assessments to identify the needs of children, young people, and families.
- Develop and coordinate multi-agency plans that improve outcomes for children.
- Deliver evidence-based interventions to support families and strengthen relationships.
- Ensure children’s voices remain central to all planning and decision-making.
- Chair and coordinate multi-agency meetings to review progress and ensure effective collaboration.
- Work closely with statutory, voluntary, community, and faith organisations to support families.
- Contribute to Child Protection processes where required.
- Maintain accurate and up-to-date case records in line with organisational policies.
- Review and update plans regularly to ensure interventions remain effective and impactful.
- Where appropriate, undertake court-related work and prepare high-quality reports.
You will bring:
- A relevant Social Work qualification (BA/MA Social Work, PG DipSW, CQSW) with Social Work England registration,
or - A Level 3 qualification in Childcare (or equivalent) with significant experience working with children and families.
- Strong knowledge of safeguarding, early intervention, and family support frameworks
- Excellent assessment and analytical skills
- The ability to build trusting relationships with families and professionals
- Strong communication and report-writing skills
- The ability to manage competing priorities in a fast-paced environment
