About the Role As a Family Practitioner, you will:

- Take on key worker responsibility, carrying out whole-family assessments and co-producing SMART, outcome-focused plans.
- Deliver direct interventions, practical support, and group work to help families overcome challenges.
- Support families in times of crisis to de-escalate situations and prevent children from entering care.
- Build strong, trusting relationships with families, engaging their wider support networks to promote sustainable change.
- Work collaboratively with Social Workers, Early Help teams, education partners, health services, and voluntary organisations to provide holistic support.
What We’re Looking For We want to hear from you if you have:
- A Level 3 qualification or above in Health, Social Care, or a related field.
- Significant experience supporting vulnerable children and families.
- Experience in assessment, planning, and delivering interventions in a multi-agency context.
- Strong knowledge of safeguarding, child development, attachment, and risk-taking behaviours.
- The ability to work flexibly, including evenings and weekends on a rota basis.

- Ongoing professional development and training.
- Flexible working between 7am and 8pm, with enhanced DBS and rota-based weekend work.
- A role where no two days are the same – you’ll be out in the community, working with families in ways that truly change lives.
