We are seeking a dynamic and compassionate Advanced Clinical Practitioner (ACP) to join our innovative Virtual Frailty Ward team. This is an exciting opportunity to deliver high-quality, patient-centred care to older adults with frailty, enabling them to receive hospital-level support in the comfort of their own homes.
As part of a multidisciplinary team, you will play a crucial role in assessing, managing, and coordinating care for patients with complex needs, helping to prevent avoidable hospital admissions and supporting safe, timely discharges.

As part of a multidisciplinary team, you will play a crucial role in assessing, managing, and coordinating care for patients with complex needs, helping to prevent avoidable hospital admissions and supporting safe, timely discharges.
- Undertake comprehensive assessments of frail patients virtually and, where necessary, in the community.
- Lead on clinical decision-making and provide expert management of acute and chronic conditions.
- Collaborate with consultants, nurses, therapists, pharmacists, and social care teams to deliver integrated care.
- Monitor patients’ progress using remote technologies and respond rapidly to changes in condition.
- Support advanced care planning, including discussions around goals of care and end-of-life planning where appropriate.
- Contribute to service development, audit, and education to ensure high standards of care and continuous improvement.
Essential Criteria:
- Registered with NMC, HCPC, or relevant professional body.
- Master’s level qualification (or equivalent experience) in Advanced Clinical Practice.
- Independent non-medical prescriber qualification.
- Proven experience in acute, community, or frailty-focused services.
- Excellent clinical assessment, decision-making, and communication skills.
- A collaborative approach and commitment to delivering person-centred care.
