Advanced Clinical Practitioner (ACP) – Virtual Frailty Ward

Location: Scarborough 
Contract: 24 Month Fixed Term Contract 

We are seeking a dynamic and compassionate Advanced Clinical Practitioner (ACP) to join our innovative Virtual Frailty Ward team. This is an exciting opportunity to deliver high-quality, patient-centred care to older adults with frailty, enabling them to receive hospital-level support in the comfort of their own homes.

As part of a multidisciplinary team, you will play a crucial role in assessing, managing, and coordinating care for patients with complex needs, helping to prevent avoidable hospital admissions and supporting safe, timely discharges.

  • Undertake comprehensive assessments of frail patients virtually and, where necessary, in the community.

  • Lead on clinical decision-making and provide expert management of acute and chronic conditions.

  • Collaborate with consultants, nurses, therapists, pharmacists, and social care teams to deliver integrated care.

  • Monitor patients’ progress using remote technologies and respond rapidly to changes in condition.

  • Support advanced care planning, including discussions around goals of care and end-of-life planning where appropriate.

  • Contribute to service development, audit, and education to ensure high standards of care and continuous improvement.

We are looking for an enthusiastic, proactive ACP who is passionate about improving outcomes for older adults living with frailty.

Essential Criteria:

  • Registered with NMC, HCPC, or relevant professional body.

  • Master’s level qualification (or equivalent experience) in Advanced Clinical Practice.

  • Independent non-medical prescriber qualification.

  • Proven experience in acute, community, or frailty-focused services.

  • Excellent clinical assessment, decision-making, and communication skills.

  • A collaborative approach and commitment to delivering person-centred care.
