Health & Social Care Officer (Adult Social Care) – Hybrid Location: Nottingham (Hybrid Working)
Contract: Full Time | 37 Hours per Week
Contract Type: Agency Contract (Likely Extension Available)

Are you passionate about making a real difference to adults in need of care and support? We're looking for an experienced Health & Social Care Officer to join a busy Adult Social Care Front Door Team, where you'll play a vital role in ensuring people receive the right support at the right time.

This is an excellent opportunity to join a fast-paced and supportive service, acting as the first point of contact for adult social care referrals and helping to safeguard vulnerable adults across the community.

The Role Working within the Adult Social Care Front Door, you'll be responsible for receiving, processing and triaging referrals from members of the public and partner agencies. This is a predominantly telephone-based role, with referrals also received through online systems.

You'll assess incoming information, gather relevant details and ensure referrals are screened appropriately before being directed to the correct service.

Your responsibilities will include:

  • Receiving and processing Adult Social Care referrals by telephone and online.
  • Screening referrals for care and support needs.
  • Triaging requests for Occupational Therapy equipment and adaptations.
  • Identifying and escalating safeguarding concerns appropriately.
  • Recording accurate information on case management systems.
  • Liaising with professionals, partner agencies and members of the public.
  • Providing a professional, empathetic and customer-focused service at all times.
About You We're looking for someone who has:

  • Previous experience within Adult Social Care.
  • Excellent communication and telephone assessment skills.
  • Knowledge of safeguarding adults and referral processes.
  • The ability to make sound decisions in a fast-paced environment.
  • Strong organisational and IT skills.
  • Experience handling sensitive and confidential information.
  • A calm, compassionate and professional approach when supporting vulnerable adults.
Working Arrangements This is a hybrid role, with approximately a 50/50 split between office and home working once fully trained.

Please note that for the first 1–2 months, you'll be expected to work full-time in the office to complete your induction and training before transitioning to hybrid working.
