Hospital Discharge Social Worker - Hackney Council

Service Care Solutions are currently recruiting for an experienced Social Worker to join the Hospital Discharge Team at Hackney Council. This is an excellent opportunity to work within a busy and supportive service, helping vulnerable adults achieve safe and effective discharges from hospital while reducing delays and improving outcomes.

Working as part of a multi-disciplinary team, you will collaborate closely with healthcare professionals, families, and community services to complete assessments, coordinate support packages, and ensure individuals receive the right level of care when returning home or moving into alternative placements.

Role Details:

• Position: Social Worker
• Team: Hospital Discharge Team
• Location: Hackney Council
• Pay Rate: £32.00 per hour Umbrella (£25.15 PAYE Exclusive)
• Contract: 3-month contract with potential extension
• Hours: Monday to Friday, 9am to 5pm
• Working Pattern: Hybrid Working
• Office Requirement: 2 days per week in the office

Key Responsibilities:

• Completing Care Act assessments and reviews
• Supporting safe and timely hospital discharges
• Arranging care packages and community support services
• Undertaking safeguarding enquiries and risk assessments
• Working closely with NHS staff, families, and external agencies
• Promoting independence and positive outcomes for service users
• Maintaining accurate and up-to-date case records

Requirements:

• Qualified Social Worker with Social Work England registration
• Experience within Adult Social Care and hospital discharge services
• Strong assessment and care planning skills
• Ability to work effectively in a fast-paced environment
• Excellent communication and partnership-working abilities
