Service Care Solutions are recruiting on behalf of a local authority for a Personal Adviser to join their Children’s Services – Care Leavers Team. This is a fantastic opportunity to make a lasting difference in the lives of young people as they transition from care to independent adulthood.
Role Overview As a Personal Adviser, you will be the key professional supporting and coordinating services for care leavers (relevant and former relevant young people). You’ll work closely with each young person to ensure their pathway plan is implemented effectively, empowering them to achieve their personal goals and build independence.

Key Responsibilities
- Provide practical and emotional advice and support to care leavers.
- Participate in assessments, pathway planning, and regular reviews post-18.
- Design and deliver tailored interventions to meet individual needs.
- Coordinate multi-agency support to ensure young people access all available services.
- Maintain accurate and up-to-date manual and electronic records in line with policy.
- Monitor progress and wellbeing, ensuring safeguarding concerns are reported promptly.
- Promote equality, inclusion, and anti-discriminatory practice at all times.
- Work flexibly, including occasional evenings and lone working as required.
- NVQ Level 3 in Children and Young People (or equivalent), or willingness to achieve within 12 months.
- Significant experience working with young people to improve outcomes.
- Knowledge of child development and safeguarding procedures.
- Strong written and verbal communication skills, including report writing.
- Experience working with young people who may be difficult to engage.
- Ability to work independently and as part of a team.
- Full UK driving licence and access to a vehicle.
- Experience supporting care leavers or working in a social care/residential setting.
- Understanding of issues affecting young people such as housing, mental health, benefits, and substance misuse.
- Knowledge of attachment, separation, and loss.
- First Aid training and awareness of equality and diversity principles.
