Location: Darby House, Lawn Central, Telford, TF3 4JA & Leavers Lounge, Meeting Point House, Southwater, Telford, TF3 4HA
Hybrid Working: Office-based 2 days per week, with flexibility depending on visits
Requirements: Full UK driving licence, access to own vehicle with business insurance
About the Role:
We are seeking a dedicated and motivated Leaving Care Personal Advisor to join our team supporting care-experienced young people. This is a vital role where you will work directly with young people aged 16–25, helping them transition from care into independent living.

About the Role:
We are seeking a dedicated and motivated Leaving Care Personal Advisor to join our team supporting care-experienced young people. This is a vital role where you will work directly with young people aged 16–25, helping them transition from care into independent living.
Key Responsibilities:
- Undertake the statutory Personal Adviser role for individual care-experienced young people from age 16 up to 21, or up to 25 where appropriate.
- Act as the key professional and case manager for Former Relevant care-experienced young people, and where needed, support Relevant or Qualifying young people.
- Work collaboratively with young people and their support networks to develop independent living skills and ensure smooth transitions to independence.
- Assist young people in preparing for leaving care, providing tailored support, advice, and guidance up to age 25.
- Commitment to empowering care-experienced young people to achieve independence.
- Ability to work both independently and as part of a supportive team.
- Full UK driving licence, access to own vehicle, and business insurance.
- Flexibility for hybrid working, with office presence generally twice per week.
