We are seeking a dedicated Outreach Family Support Worker to join our Early Help team. In this role, you will take the lead in coordinating support for families by working closely with our multi-agency partners to ensure a whole-family approach to meeting identified needs.

Key Responsibilities
- Undertake Early Help Assessments with children, young people, and their families.
- Develop and implement support plans tailored to individual family circumstances.
- Chair Team Around the Family (TAF) meetings to review progress and identify additional areas of support.
- Provide direct support and interventions to children and young people, exploring their views and worries to ensure their voices shape the help they receive.
- Work collaboratively with a range of partners to promote positive change and improve long-term outcomes.
- Passionate about improving the lives of children, young people, and families.
- Experienced in family support or similar community-based roles.
- Skilled in engaging families using a strength-based, empowering approach.
- Confident in multi-agency working and coordinating support plans.
- Able to communicate effectively with children, families, and professionals.
