Hours: 35 hours per week
Contract: Initial 3-month period (with the possibility of extension)

About the Role
Liverpool Early Help is committed to delivering empowering, strength-based interventions that enable children, young people, and families to make positive and lasting changes in their lives.
We are seeking a dedicated Outreach Family Support Worker to join our Early Help team. In this role, you will take the lead in coordinating support for families by working closely with our multi-agency partners to ensure a whole-family approach to meeting identified needs.

Key Responsibilities

• Undertake Early Help Assessments with children, young people, and their families.
• Develop and implement support plans tailored to individual family circumstances.
• Chair Team Around the Family (TAF) meetings to review progress and identify additional areas of support.
• Provide direct support and interventions to children and young people, exploring their views and worries to ensure their voices shape the help they receive.
• Work collaboratively with a range of partners to promote positive change and improve long-term outcomes.

About You We’re looking for someone who is:

• Passionate about improving the lives of children, young people, and families.
• Experienced in family support or similar community-based roles.
• Skilled in engaging families using a strength-based, empowering approach.
• Confident in multi-agency working and coordinating support plans.
• Able to communicate effectively with children, families, and professionals.
