Key Responsibilities As an Outreach Team Leader, you will:
- Deliver high-quality, safe, person-centred services that focus on wellbeing, promoting independence, and supporting healthy lifestyles — ensuring that choice, rights, and control are at the heart of all practice.
- Provide comprehensive support to social care customers, ensuring their individual needs are met effectively.
- Work collaboratively as part of the management team to contribute to the development and smooth running of services in line with local authority policies, procedures, and relevant legislation.
- Deputise for the manager in their absence and take responsibility for specific areas of the service under their direction.
- Manage, support, and develop staff within a robust performance management framework.
- Encourage creativity, innovation, and flexibility in responding to the changing needs of service users.
- Ensure all services comply with CQC fundamental standards and promote continuous improvement.
- Deliver services in accordance with each individual’s Assessment, Support Plan, and Risk Assessments.
About you:
- Equivalent vocational experience of NVQ level 4 or above
- Knoweldge and experience of delivering and reviewing personalised services that focus on independacne, wellbeing and are outcome focused to vulnerable adults with a range of care and support needs including behaviour that challenges.
- Ability to supoprt staff through supervision including setting targets and monitoring individual performance against service objectives and competences
- Ability to manage own work load, be flexible, work with others within the team to meet deadlines and be pro-active in meeting the needs of the service and stakeholders
- Knowledge of current legislation around Adult Social Care including Safeguarding.
- Excellent IT skills and an ability to produce reports and letters
- Ability to cover out of hours on call on a rota basis
