Job Title: Personal Advisor – Leaving Care Service
Location: Hackney 
Salary: £24 UMB (£22.18 PAYE)
Contract: Full-time | Initial 3 month contract - potential to extend

About the Role We are seeking a dedicated and compassionate Personal Advisor to join our Leaving Care Service, supporting care leavers aged 18–25 as they transition into independent living. This is an exciting opportunity to make a lasting difference in the lives of young people, helping them access the right support, housing, education, and employment opportunities.
You will work as part of a multi-disciplinary Unit Model, under the supervision of a Consultant Social Worker or Practice Development Manager, leading on individual cases to develop and review Pathway Plans that identify needs, set achievable goals, and promote positive outcomes.
This role requires a persistent, hands-on, and relationship-based approach — working directly with young people, families, and partner agencies to empower care leavers to thrive independently.

Key Responsibilities
  • Build trusting, respectful relationships with care leavers, their families, and support networks.
  • Lead on the creation and review of Pathway Plans, ensuring each reflects the care leaver’s voice, needs, and aspirations.
  • Work collaboratively with partner agencies to coordinate holistic support and ensure all professional perspectives are reflected in planning.
  • Provide practical and emotional support to care leavers in developing independent living skills, financial literacy, and tenancy management.
  • Identify and report any concerns about accommodation quality, safety, or wellbeing to line managers promptly.
  • Encourage engagement in education, training, or employment opportunities, using a strengths-based and task-centred approach.
  • Maintain accurate, up-to-date case records and contribute to assessments of need and risk.
  • Participate in multi-agency meetings and reviews to promote best outcomes for young people.
  • Demonstrate a flexible approach, including occasional evening or weekend work, in response to care leavers’ needs.
  • Take responsibility for personal professional development through supervision, training, and reflective practice.
About You You will bring enthusiasm, empathy, and resilience to the role, with a genuine commitment to supporting young people leaving care. You should have:
  • Experience working with young people or vulnerable groups in a social care or support setting.
  • Strong communication and relationship-building skills, with the ability to engage positively in challenging situations.
  • Knowledge of Pathway Planning, safeguarding, and relevant care leavers’ legislation.
  • Confidence in working independently and collaboratively within a multi-agency framework.
  • Excellent organisational and record-keeping skills
  • A proactive, reflective approach to professional development and service improvement.
  • NVQ Level 3 or equivalent child and family training.
