Job Title: Qualified Social Worker (Adults)

Location: Bournemouth and Poole

Contract Length: 24 months

Are you a dedicated and experienced Qualified Social Worker (Adults) looking for a new opportunity to make a difference in the Bournemouth and Poole area? Our client is seeking a qualified professional to join their high-demand hospital discharge team.

Candidate Responsibilities:

• Conduct assessments and reviews to support safe and effective discharge processes from acute and community hospitals.
• Collaborate with multidisciplinary teams to ensure the well-being of vulnerable adults.
• Maintain accurate and timely case notes and reports.
• Participate in regular team meetings and contribute to the development of care plans.
• Provide support and guidance to service users and their families.
