Reablement Assistant 
35 hours per week | Initial 12-Week Post

We are seeking a committed and compassionate Reablement Assistant to join our team on an initial 12-week placement. This role is key in supporting service users to regain confidence, independence, and daily living skills, enabling them to remain safely in their own homes.

About the Role
As a Reablement Assistant, you will work directly with individuals in their homes, delivering person-centred support in line with agreed care and support plans. You will be part of an integrated team working alongside allied health and social care professionals.

Main Responsibilities
  • Support service users to meet their physical, social, and emotional needs, promoting independence and wellbeing.
  • Assist with agreed tasks within individual support and care plans.
  • Work with individuals to improve and maintain daily living skills, using appropriate equipment when required.
  • Liaise and work alongside other professionals to minimise risk and support people to remain at home.
  • Contribute to assessments and reviews and support the implementation of person-centred plans.
  • Promote good care practice, including dignity, independence, empowerment, and respect for cultural, spiritual, and religious needs.
  • Maintain accurate records and report any significant changes to the Line Manager.
  • Attend supervision sessions and work collaboratively as part of a wider team.
  • Support service users to engage in activities and recreational pursuits.
  • Be flexible and responsive to the changing needs of the service.
  • Carry out all duties in line with City Council policies and procedures, including equality, diversity, health and safety, and anti-discriminatory practice.
Working Pattern
  • 35 hours per week 
Essential Requirements
  • Up-to-date face-to-face Moving & Handling training (certificate required).
  • Right to work in the UK – sponsorship is not available.
  • Willingness to work a 35-hour post only (no part-time or overtime hours available).
