About the Role
Southampton City Council is seeking compassionate and motivated Reablement Assistants to provide hands-on care and support to residents across the city. Using a strengths-based approach, you will help people achieve their reablement goals, regain independence, and improve their health and wellbeing.
You will work across a 7-day service, providing care within operational hours 6:30am – 10:00pm, with visits to residents across Southampton.

Key Responsibilities
- Provide advice, guidance, and support on community services and adult social care to help people maintain independence, health, and wellbeing.
- Enable people to identify their assets, networks, and aspirations, connecting them to wider opportunities and increasing confidence and self-esteem.
- Support people with personal care, mobility, nutrition, skin integrity, and medication management [training provided].
- Encourage people to graduate from dependence to independence in daily living and social living tasks.
- Conduct timely reviews of care, support, and occupational therapy plans, ensuring people reach their potential.
- Use appropriate techniques to support people with communication difficulties and manage anxiety.
- Maintain accurate documentation in line with Medication Administration Records (MAR) and other care records.
We are looking to cover the following shifts across Monday to Sunday:
- 2 x 07:00 – 13:00
- 1 x 08:00 – 14:00
- 2 x 17:00 – 22:00
- 1 x 16:00 – 21:00
Person Specification
- Experience in adult social care or reablement is desirable but not essential; full training will be provided.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Commitment to promoting independence, dignity, and wellbeing of residents.
