Reablement Worker
Location: Hammersmith, London
Rate: £20 per hour (via Umbrella) (£15 PAYE an hour)
Hours: 36 hours per week (AM/PM and weekend shifts required)
ontract: 3 months initially, with potential for extension
Role Overview Hammersmith & Fulham Council are seeking a dedicated and compassionate Reablement Worker to join their Adult Social Care Reablement Service. This team helps adults regain confidence and independence after illness, surgery, or reduced mobility—supporting them to live safely and independently at home.
You will work closely with therapists, coordinators, and other health and social care professionals to deliver reablement programmes and support residents in achieving their personal goals.

Key Responsibilities
- Deliver personalised care and reablement support within residents’ homes.
- Support residents with daily living activities to promote independence.
- Implement and monitor individual reablement plans under supervision of qualified professionals.
- Encourage residents to achieve recovery and mobility goals, ensuring dignity and respect.
- Liaise with families, carers, and health professionals to ensure continuity of care.
- Maintain accurate and timely records in line with Council procedures.
- Work flexibly across AM, PM, and weekend shifts as required by the service.
- Level 3 Diploma in Health and Social Care (or willingness to work towards it).
- Care Certificate (or willingness to complete).
- Experience providing care and support within health or social care settings.
- Understanding of reablement and person-centred care principles.
- Strong communication, observation, and record-keeping skills.
- Ability to work independently and as part of a multidisciplinary team.
- A flexible, empathetic, and professional approach to resident care.
- You must be willing to undertake an Enhanced DBS check.
- Training and supervision provided.
- Immediate start available.
