Contract: Initial 6-month assignment
Hours: 37 hours per week
Pay: £20 per hour (via umbrella company) (£15 PAYE an hour)
About the Role Gloucestershire County Council is seeking two Rehabilitation Services Coordinators to join our dedicated, multi-disciplinary team at Great Western Court in Gloucester. Our centre provides comprehensive rehabilitation and 24-hour care for individuals following a hospital discharge or admission from the community — supporting them to regain independence and live fulfilling, dignified lives.
In this rewarding role, you will play a key part in coordinating personalised rehabilitation programs, leading shifts, and supporting a team of Rehabilitation Assistants to deliver exceptional care.

Key Responsibilities
- Lead shifts and coordinate rehabilitation and care programmes tailored to individual needs.
- Safely administer, prompt, or assist with medication in line with care plans.
- Support residents with mobility, personal care, and daily living tasks.
- Collaborate with the therapy and nursing teams to develop, review, and implement care plans and risk assessments.
- Maintain accurate written and electronic records.
- Facilitate smooth admissions and discharges, promoting recovery and independence.
- Create a positive, caring, and stimulating environment for all residents.
- Experience in a rehabilitation, care, or health support setting.
- Strong leadership skills and confidence in leading shifts.
- Excellent communication and teamwork abilities.
- A compassionate, patient, and proactive approach to supporting individuals.
- The ability to manage records and follow clinical or care procedures accurately.
