Key Responsibilities:
- Review Care Plans: Conduct scheduled reviews of care and support plans to ensure outcomes are met and services remain appropriate.
- Engage with Service Users: Communicate with service users, carers, and providers to gather feedback and assess satisfaction.
- Monitor Service Delivery: Ensure commissioned services are delivered as agreed and meet quality standards.
- Identify Changes in Needs: Highlight cases where needs have changed significantly for reassessment by qualified social workers.
- Record Keeping: Maintain accurate and timely records on the Council’s case management system.
- Compliance: Ensure all activities comply with the Care Act 2014, safeguarding principles, and Hackney Council procedures.
- Collaboration: Work closely with social workers, providers, and commissioning officers to drive continuous improvement in service delivery.
- Reporting: Provide data and insights to support commissioning decisions and performance monitoring.
- Strong organisational and communication skills
- Attention to detail and the ability to manage multiple cases
- Commitment to improving outcomes for adults receiving care services
- Knowledge of the Care Act 2014 and adult safeguarding principles is desirable
