Reviewing Officer – Adult Social Care (Hospital Team)
Location: Hackney
Rate:£29 UMB per hour
Contract: Agency - 3 Months - potential to extend
Hours: Full-time - 36 hours per week

Role Overview We are recruiting a Reviewing Officer to support the local authority of Hackney to work within an Adult Social Care Hospital Team, supporting safe and timely discharges. You’ll complete assessments and reviews, coordinate care packages, manage risk, and work closely with health and social care partners in a fast-paced hospital setting.
Key Duties

• Complete assessments and reviews under the Care Act 2014
• Develop and review care plans with individuals, families, and MDTs
• Support and lead discharge planning, including D2A pathways
• Identify and respond to safeguarding concerns
• Act as an advocate to ensure person-centred outcomes
• Liaise with hospital staff, community teams, and external providers
• Identify and manage discharge-related risks

Essential Requirements

• Adult Social Care experience within a hospital or discharge team
• Strong assessment, communication, and organisational skills
• Knowledge of the Care Act 2014 and Mental Capacity Act
• Ability to manage a busy caseload in a pressured environment
