Make a Real Difference to Children, Young People and Families in Staffordshire Service Care Solutions are working in partnership with Staffordshire County Council to recruit a passionate and skilled SEND Key Worker to join the Education, Health & Care Assessment & Planning Team. This is a fantastic opportunity to play a central role in transforming the lives of children, young people, and adults with Special Educational Needs and Disabilities (SEND).

About the Role As a SEND Key Worker, you will coordinate and facilitate Education, Health & Care (EHC) assessments and support the development of high-quality, outcome-focused EHC Plans. You will act as a consistent point of contact for individuals with SEND and their families—bringing together professionals across education, health, social care and partner agencies to ensure needs are understood and support is well coordinated.
This role is essential in promoting independence, empowering families, and ensuring the best possible outcomes for children and young people.
Key Responsibilities ? Build strong, collaborative and respectful relationships with individuals with SEND and their families.
? Coordinate multi-agency assessments and develop outcome-focused EHC Plans.
? Analyse information to understand needs, strengths and risks, identifying strategies to manage or reduce those risks.
? Facilitate support from key professionals, including Social Care, Educational Psychology and Health.
? Explore innovative support options and enable informed choice for families.
? Broker appropriate services, education providers, and support packages.
? Understand and negotiate costs related to support over time.
? Manage and monitor personal budgets and ensure families understand contributions.
? Maintain accurate records, reports and performance data.
? Coordinate annual reviews and use evidence to update plans and arrangements.
? Provide short-term intervention and monitoring where needed.
? Work in line with the SEND Code of Practice and contribute to continuous service improvement.
? Follow safeguarding, health & safety, equalities, climate change and financial procedures.

About You We’re looking for someone who is passionate, resilient, and driven by improving outcomes for children and families.
You will ideally have:
- NVQ Level 3 or equivalent in education, social care, childcare, guidance, or a related field (or willingness to work toward this).
- Strong experience supporting children, young people or adults with disabilities.
- Solid understanding of SEND, child development, family dynamics and multi-agency working.
- Knowledge of safeguarding procedures, the Children Act, Education Act, Mental Capacity Act, and relevant SEND legislation.
- Ability to analyse complex information and communicate effectively with families and professionals.
- Excellent organisational skills with the ability to prioritise and manage a varied caseload.
- Confident IT and report-writing skills.
- Conflict resolution skills, creativity and a solution-focused mindset.
- A full UK driving licence (reasonable adjustments considered).
