We’re recruiting a SEND Key Worker to join a well-established SEND Education, Health & Care Assessment and Planning Team, working as part of the wider Families First – Targeted Services offer across Staffordshire.
About the role As a SEND Key Worker, you’ll play a vital role in co-ordinating Education, Health & Care (EHC) assessments and plans, ensuring they are person-centred, outcome-focused and fully compliant with SEND legislation and the Code of Practice.
You’ll act as the key point of contact for children, young people and adults with SEND and their families, working collaboratively with professionals across education, health, social care and partner agencies to deliver the right support at the right time.

You’ll act as the key point of contact for children, young people and adults with SEND and their families, working collaboratively with professionals across education, health, social care and partner agencies to deliver the right support at the right time.
Key responsibilities
- Act as a Key Worker throughout the EHC assessment and planning process
- Co-ordinate multi-agency assessments and contributions (education, health, social care, psychology and more)
- Build strong, respectful relationships with individuals with SEND and their families
- Analyse information to identify needs, strengths, risks and achievable outcomes
- Broker and coordinate services, support packages and education provision
- Support personal budgets, funding allocations and reviews
- Lead and co-ordinate annual reviews of EHC Plans
- Maintain accurate electronic case records and performance data
- Work in line with safeguarding, equality, health & safety and SEND legislation
- Contribute to service development, quality assurance and continuous improvement
- NVQ Level 3 (or equivalent) in a relevant field or willingness to work towards this
- Strong experience supporting children, young people or adults with disabilities
- Sound understanding of SEND legislation, safeguarding and multi-agency working
- Excellent communication, organisation and case management skills
- Ability to work creatively, manage risk positively and problem-solve
- Confident using IT systems for records, reports and data
- A resilient, curious and values-driven professional who genuinely wants to make a difference
- Ability to travel across Staffordshire (reasonable adjustments considered)
