This role is crucial in supporting children and young people with Special Educational Needs and Disabilities (SEND), providing person-centred support and helping families access the right services.
Key Responsibilities
- Manage a caseload of children and young people with SEND.
- Coordinate and facilitate assessments and develop outcome-focused EHC plans.
- Conduct annual reviews in line with statutory requirements.
- Work collaboratively with families, schools, social care, and health professionals.
- Maintain accurate records, data, and documentation in line with policies and procedures.
- Identify, negotiate, and coordinate support packages, including personal budgets where applicable.
- Ensure compliance with safeguarding legislation, SEND Code of Practice, and relevant policies.
- Experience working with children or young people with SEND or disabilities.
- Knowledge of SEND legislation, statutory EHC processes, and safeguarding requirements.
- Strong communication, organisational, and record-keeping skills.
- Ability to work independently and collaboratively across multi-agency teams.
- NVQ3 or equivalent in education, social care, child care, or a related discipline (or willingness to undertake).
