Service Care Solutions are currently recruiting for an experienced Social Care Assessor to join the Adults Health & Integration service within Cheshire East Council.
This is an excellent opportunity for a passionate and experienced social care professional to make a genuine difference to vulnerable adults and their carers, while working within a supportive multi-disciplinary environment.
As a Social Care Assessor, you will be responsible for completing needs-based assessments, identifying risks and presenting needs, developing and coordinating individual Care Plans, and managing a caseload to ensure people receive the right support at the right time.

Service Care Solutions are currently recruiting for an experienced Social Care Assessor to join the Adults Health & Integration service within Cheshire East Council.
This is an excellent opportunity for a passionate and experienced social care professional to make a genuine difference to vulnerable adults and their carers, while working within a supportive multi-disciplinary environment.
As a Social Care Assessor, you will be responsible for completing needs-based assessments, identifying risks and presenting needs, developing and coordinating individual Care Plans, and managing a caseload to ensure people receive the right support at the right time.
The Role You will:
- Complete needs-based assessments with vulnerable adults and their carers.
- Identify individual needs, risks and the urgency of required support.
- Develop, implement and coordinate personalised Care Plans.
- Act as Care Manager for an allocated caseload.
- Monitor, review and amend Care Plans in response to changing circumstances.
- Work closely with clients, families, carers and a range of professionals and agencies.
- Liaise with statutory and voluntary organisations to coordinate effective care and support.
- Undertake financial assessments and explain the financial implications of care services.
- Support clients to access appropriate benefits and maximise available income.
- Maintain accurate, timely and comprehensive electronic and written records.
- Work collaboratively within a multi-disciplinary environment to achieve positive outcomes.
You should have:
- NVQ Level 3 in Social Care or equivalent experience as a minimum.
- Experience completing assessments and/or managing care packages.
- Experience working with vulnerable adults and their carers.
- A strong understanding of safeguarding and risk assessment.
- Knowledge of relevant legislation, care standards and statutory requirements.
- The ability to identify needs and source appropriate services and resources.
- Excellent communication, negotiation and interpersonal skills.
- Experience working alongside other professionals, agencies and voluntary organisations.
- Strong organisational and IT skills, with the ability to manage a caseload and meet deadlines.
- A sensitive, professional and person-centred approach.
- The ability to work effectively as part of a multi-disciplinary team.
- Flexibility and willingness to travel across Cheshire East when required.
