Social Care Facilitator – Adult Social Care Walsall | Hybrid Working | Full-Time | Initial 3-Month Contract
Are you passionate about supporting vulnerable adults to live independent, fulfilling lives? Do you have experience within Adult Social Care and enjoy working as part of a collaborative, multi-disciplinary team?
Service Care Solutions are recruiting for a Social Care Facilitator to join a busy Adult Social Care team in Walsall. This is an excellent opportunity to make a genuine difference by supporting adults and their carers through assessment, care planning and person-centred support.
The Role As a Social Care Facilitator, you will play a key role in assessing needs, supporting care planning and empowering individuals to make informed choices about their care. Working within a multi-skilled team, you will help people maximise their independence while promoting wellbeing and access to community resources.
This is a hybrid position, with a minimum of two days per week based in the office, offering a great balance between office and home working.

Key Responsibilities
- Support the assessment of vulnerable adults and their carers, contributing to person-centred care planning.
- Promote independence, wellbeing, choice and control in line with the principles of self-directed support.
- Undertake allocated care management duties.
- Build positive relationships with service users, families and carers through effective communication.
- Work collaboratively with colleagues across Adult Social Care and multi-disciplinary teams.
- Support individuals to access community services and resources that help them remain independent.
- Identify the need for assistive equipment and adaptations, arranging or referring for appropriate provision following training.
- Participate in ongoing professional and team development.
- Has experience working within Adult Social Care or a similar health and social care setting.
- Has knowledge of assessment and care planning processes.
- Can communicate effectively with vulnerable adults, carers and partner agencies.
- Is passionate about delivering person-centred support.
- Works well independently and as part of a team.
- Holds a full UK driving licence and has access to transport for work purposes.
- Full-time position (37 hours per week)
- Hybrid working arrangements
- Initial 3-month contract with potential for extension
