Social Care Facilitator Adult Social Care | Walsall Full-Time | Hybrid Working | Immediate Opportunity

Service Care Solutions are currently recruiting for a Social Care Facilitator to join an Adult Social Care & Health team in the Walsall area.

This is an excellent opportunity for an experienced social care professional to join a multi-disciplinary team supporting vulnerable adults, helping individuals achieve greater independence, choice and control over their lives.

The Role As a Social Care Facilitator, you will work alongside colleagues across health and social care to support the assessment, care management and wider support needs of vulnerable adults.

You will take a person-centred and strengths-based approach, working with individuals, families and carers to identify appropriate support and enable people to make informed decisions about their care and wellbeing.

Key Responsibilities
  • Contribute to effective support planning, promoting choice, independence and wellbeing.
  • Assist with the assessment of the needs of vulnerable adults and their carers.
  • Support individuals to exercise choice and control in line with self-directed support principles.
  • Undertake allocated care management duties.
  • Communicate effectively with service users, families and carers using a person-centred approach.
  • Work closely with colleagues within multi-disciplinary teams.
  • Liaise with community organisations and other agencies to help people access appropriate resources and services.
  • Promote independence and support individuals to remain active within their communities.
  • Participate in team development and ongoing learning.
  • Identify appropriate assistive equipment and adaptations where training and guidance allows.
  • Make referrals to relevant specialist services where more complex equipment or adaptations are required.
What We're Looking For We're looking for someone who has a good understanding of adult social care and experience working with vulnerable adults.

You will need:

  • Experience within a social care, health or community-based environment.
  • A strong understanding of person-centred support.
  • Excellent communication and interpersonal skills.
  • The ability to work collaboratively within a multi-disciplinary team.
  • An understanding of promoting independence, choice and wellbeing.
  • The ability to manage allocated responsibilities effectively.
  • A full UK driving licence.
Working Arrangements ? Location: Walsall
? Hybrid working: Minimum of 2 days office-based per week
? Hours: 37 hours per week
? Working hours: Monday–Friday, 9:00am–5:30pm
? Initial contract: 7 September 2026 – 7 December 2026
