Social Care Facilitator Adult Social Care | Walsall Full-Time | Hybrid Working | Immediate Opportunity
Service Care Solutions are currently recruiting for a Social Care Facilitator to join an Adult Social Care & Health team in the Walsall area.
This is an excellent opportunity for an experienced social care professional to join a multi-disciplinary team supporting vulnerable adults, helping individuals achieve greater independence, choice and control over their lives.
The Role As a Social Care Facilitator, you will work alongside colleagues across health and social care to support the assessment, care management and wider support needs of vulnerable adults.
You will take a person-centred and strengths-based approach, working with individuals, families and carers to identify appropriate support and enable people to make informed decisions about their care and wellbeing.
Key Responsibilities
You will need:
? Hybrid working: Minimum of 2 days office-based per week
? Hours: 37 hours per week
? Working hours: Monday–Friday, 9:00am–5:30pm
? Initial contract: 7 September 2026 – 7 December 2026

Key Responsibilities
- Contribute to effective support planning, promoting choice, independence and wellbeing.
- Assist with the assessment of the needs of vulnerable adults and their carers.
- Support individuals to exercise choice and control in line with self-directed support principles.
- Undertake allocated care management duties.
- Communicate effectively with service users, families and carers using a person-centred approach.
- Work closely with colleagues within multi-disciplinary teams.
- Liaise with community organisations and other agencies to help people access appropriate resources and services.
- Promote independence and support individuals to remain active within their communities.
- Participate in team development and ongoing learning.
- Identify appropriate assistive equipment and adaptations where training and guidance allows.
- Make referrals to relevant specialist services where more complex equipment or adaptations are required.
You will need:
- Experience within a social care, health or community-based environment.
- A strong understanding of person-centred support.
- Excellent communication and interpersonal skills.
- The ability to work collaboratively within a multi-disciplinary team.
- An understanding of promoting independence, choice and wellbeing.
- The ability to manage allocated responsibilities effectively.
- A full UK driving licence.
