We are currently recruiting for a CWD Short Breaks Coordinator to provide sickness cover within a dedicated and supportive Short Breaks team. This is a fantastic opportunity to step into a rewarding role where your work will have a direct and meaningful impact.

The Role As a Short Breaks Coordinator, you will manage an allocated caseload, working closely with children and their families to:

• Assess eligibility and need for short breaks
  
  
• Identify and secure appropriate support provision
  
  
• Regularly review plans in line with regulatory requirements
  
  
• Ensure services are delivering positive outcomes for children and families
  
  

You will play a key role in ensuring that every short breaks plan is effective, appropriate, and tailored to individual needs.

What We’re Looking For We’re looking for someone who is:

• Experienced in working with children with disabilities and their families
  
  
• Skilled in carrying out assessments (e.g. Early Help Assessments)
  
  
• A confident communicator with strong relationship-building skills
  
  
• Able to work restoratively with families
  
  
• Comfortable working collaboratively with partners across education, health, and social care
