👶 Your Role Will Include:

• Assessing and addressing the needs of children, young people, and their families
• Creating tailored care plans to support well-being and inclusion
• Collaborating with education, health, and care professionals
• Conducting home visits, assessments, and risk reviews
• Offering guidance and support to parents, carers, and service users
• Coordinating with partner agencies for effective service delivery
