About the Role – Social Worker (Adults)

As a Social Worker (Adults), you will be instrumental in assessing, planning, and providing essential support to adults in need within our community. Your expertise will help promote independence and improve quality of life for service users. This contract offers a chance to work within a dynamic team, delivering impactful services to those who need it most.

Candidate Responsibilities – Social Worker (Adults)

• Conduct comprehensive assessments of adults' social care needs.
• Develop tailored care plans in collaboration with service users and their families.
• Provide advice and advocacy to ensure individuals access appropriate services.
• Liaise with healthcare professionals, families, and external agencies.
• Maintain accurate and timely case records in compliance with policies.
• Support and empower adults to achieve greater independence.
• Attend multi-disciplinary team meetings and contribute to care reviews.
• Ensure all safeguarding procedures are followed diligently.
• Complete and update documentation regarding DBS checks and compliance, including identifying the correct level of DBS (Enhanced – including barred for both children’s and adults, as required).

Candidate Requirements

• Relevant social work qualification (e.g., Degree in Social Work).
• Valid registration with Social Work England.
• Experience working with adult social care teams.
• Knowledge of safeguarding procedures and legislation.
• Ability to complete detailed assessments and reports.
• Commitment to service users' well-being and safety.
• Valid DBS check, preferably enhanced including barred for both children’s and adults. Please confirm the specific DBS requirements for this role.
