🔎 The Role As a Senior Social Worker, you will:
- Complete Care Act assessments, develop person-centred support plans, and carry out regular reviews.
- Promote independence, manage risk, and uphold individuals’ rights and choices.
- Lead on safeguarding enquiries and undertake Mental Capacity Act (MCA) assessments.
- Work collaboratively with families, health professionals, and care providers to coordinate effective support.
- Respond to crisis situations, including hospital discharge planning and assessing eligibility for services such as NHS Continuing Healthcare.
- Maintain accurate case records, work within key legal frameworks (including the Care Act and MCA), and actively participate in supervision, reflective practice, and ongoing CPD.
