Agency Social Worker – Learning Disability Team (West)
Location: Rhondda Area (West Borough)
Organisation: Rhondda Cynon Taf Council
Contract: Initially 3 months- full time - 37 hours per week
Working Pattern: Remote working with 1 office day per week
Are you an experienced agency social worker with a passion for supporting adults with learning disabilities? We are working with Rhondda Cynon Taf Countyr Borough Council and are seeking a skilled practitioner to join the West Learning Disability Team, working across the Rhondda area within a well-established and supportive service.
This is an excellent opportunity to contribute to high-quality, strengths-based social work practice while working within a collaborative multidisciplinary environment.
About the Role
You will work with adults with learning disabilities, delivering effective assessment, care planning, and review functions to safeguard and promote wellbeing. You will manage a varied caseload and work closely with UK legislation and best practice frameworks to ensure positive, person-centred outcomes.
The team operates primarily remotely, with one office-based day per week, and offers a supportive team culture with experienced colleagues.

Key Responsibilities
- Safeguard and promote the wellbeing of adults through high-quality social work practice.
- Undertake assessments, care planning, and reviews in line with the Social Services and Well-Being (Wales) Act 2014.
- Develop and monitor care and support plans based on identified outcomes.
- Complete Mental Capacity Assessments and work within Mental Capacity Act principles.
- Assess and manage risk in a balanced way that promotes co-production, independence, and choice.
- Identify and build on individual strengths and capabilities.
- Work collaboratively with partner agencies using a multidisciplinary approach.
- Maintain accurate and timely electronic case records, assessments, plans, and reviews in line with policy and procedure.
- Qualified Social Worker with relevant registration.
- Experience working with adults with learning disabilities.
- Strong background in assessment, care planning, and review.
- Experience completing Mental Capacity Assessments.
- Confident working in multi-agency settings.
- Ability to travel independently across the service area and live within reasonable commuting distance.
