Are you passionate about making a difference in adults' lives through expert care and support? We are excited to announce multiple opportunities for Social Worker (Adults, Care and Support) positions within a newly created team operating on a countywide basis. This is an excellent chance to be part of a dynamic team dedicated to responding swiftly to referrals and providing vital assessments and support.
As a Social Worker (Adults, Care and Support), you will play a crucial role in delivering high-quality care assessments across Cornwall. Your responsibilities will include carrying out Care Act assessments, care and support planning, and Mental Capacity assessments, ensuring individuals receive the support they need. Working autonomously, you will manage a caseload within designated geographical areas, adapting to flexible working hours while adhering to strict deadlines.

Candidate Responsibilities – Social Worker (Adults, Care and Support)
- Conduct Care Act assessments and associated reviews to evaluate client needs
- Develop person-centered care and support plans tailored to individual requirements
- Perform Mental Capacity assessments and other related evaluations
- Respond promptly to referrals across different geographical locations as per assessed needs
- Work independently, demonstrating flexibility to adapt to varying scheduled visits
- Maintain accurate documentation and records in compliance with statutory regulations
- Collaborate with other teams and professionals to ensure comprehensive support
- Proven experience in conducting Care Act assessments and support planning
- Ability to perform Mental Capacity assessments and related evaluations
- Access to a car and the ability to undertake visits across Cornwall
- Flexibility to work in different geographical areas on a countywide basis
- Self-motivated, autonomous working style with excellent time management skills
- Comfortable working remotely and managing strict deadlines
- Must be able to start from the 1st of April, 2026, with an earlier start considered preferred
- Contract length: 3 months initial contract with the possibility of extension
