Mental Health Social Worker - CMHT - Swansea Council
Service Care Solutions are currently recruiting for an experienced Mental Health Social Worker to join the Community Mental Health Team (CMHT) at Swansea Council on a 3-month ongoing agency contract.
This role offers the opportunity to work within a multidisciplinary team supporting adults with a range of mental health needs. The successful candidate will be responsible for managing a caseload, undertaking assessments, care planning, reviews, risk assessments, safeguarding enquiries, and coordinating support to help individuals achieve positive outcomes within the community.

Role Details:
- Location: Swansea
- Team: Community Mental Health Team (CMHT)
- Contract: 3 months ongoing
- Hours: Monday to Friday, 9:00am to 5:00pm
- Pay Rate: £38.43 per hour Umbrella
- Pay Rate: £30.50 per hour PAYE (excluding holiday pay)
- Manage a caseload of adults with mental health needs.
- Undertake Care and Support Assessments and reviews.
- Complete risk assessments and safeguarding work where required.
- Develop and review support plans to promote independence and recovery.
- Work closely with health professionals, families, and partner agencies.
- Maintain accurate and timely case records.
- Qualified Social Worker.
- Registered with Social Care Wales.
- Previous experience working within a Mental Health Team or CMHT.
- Knowledge of relevant mental health and safeguarding legislation.
- Ability to work effectively within a multidisciplinary environment.
