Mental Health Social Worker - CMHT - Swansea Council

Service Care Solutions are currently recruiting for an experienced Mental Health Social Worker to join the Community Mental Health Team (CMHT) at Swansea Council on a 3-month ongoing agency contract.

This role offers the opportunity to work within a multidisciplinary team supporting adults with a range of mental health needs. The successful candidate will be responsible for managing a caseload, undertaking assessments, care planning, reviews, risk assessments, safeguarding enquiries, and coordinating support to help individuals achieve positive outcomes within the community.

Role Details:

• Location: Swansea
• Team: Community Mental Health Team (CMHT)
• Contract: 3 months ongoing
• Hours: Monday to Friday, 9:00am to 5:00pm
• Pay Rate: £38.43 per hour Umbrella
• Pay Rate: £30.50 per hour PAYE (excluding holiday pay)

Key Responsibilities:

• Manage a caseload of adults with mental health needs.
• Undertake Care and Support Assessments and reviews.
• Complete risk assessments and safeguarding work where required.
• Develop and review support plans to promote independence and recovery.
• Work closely with health professionals, families, and partner agencies.
• Maintain accurate and timely case records.

Requirements:

• Qualified Social Worker.
• Registered with Social Care Wales.
• Previous experience working within a Mental Health Team or CMHT.
• Knowledge of relevant mental health and safeguarding legislation.
• Ability to work effectively within a multidisciplinary environment.
