Candidate Responsibilities:
- Conduct assessments to determine the needs of individuals and families requiring social care support
- Develop and implement care plans tailored to the specific needs of each service user
- Work collaboratively with other professionals, such as healthcare providers and community services, to ensure comprehensive support
- Provide advice and guidance to service users and their families on available support services
- Maintain accurate and up-to-date case notes and records in accordance with data protection regulations
- Competitive pay rate of £39 per hour umbrella inclusive of holiday pay Paye £31.32 exlusive of holiday pay
