Our client, a well-established social care organization based in South - Fawcett House, is currently seeking a dedicated and experienced Social Worker - Assessment Team to join their team on a contract basis. This role offers a competitive pay rate of £39 per hour umbrella and provides a hybrid working environment.

Candidate Responsibilities:

• Conduct assessments to determine the needs of individuals and families requiring social care support
• Develop and implement care plans tailored to the specific needs of each service user
• Work collaboratively with other professionals, such as healthcare providers and community services, to ensure comprehensive support
• Provide advice and guidance to service users and their families on available support services
• Maintain accurate and up-to-date case notes and records in accordance with data protection regulations

Required Skills:

• Qualified Social Worker with relevant experience in the assessment team setting
• Registration with Social Work England
• Strong communication and interpersonal skills
