Social Worker – Community Mental Health Team Location: Central Clinic, Orchard Street, Swansea
Contract: Full-time (37 hours per week)
Hybrid working: Combination of office, home-based, and community visits
Description
The City and County of Swansea are seeking a dedicated and experienced Social Worker to join the Community Mental Health Team (CMHT). This is an exciting opportunity to make a real difference in the lives of individuals experiencing serious mental health challenges, helping them to live as independently and meaningfully as possible within the community.
You’ll be part of a multi-disciplinary, recovery-focused team, working collaboratively with health professionals, carers, and families to provide integrated and person-centred care.

You’ll be part of a multi-disciplinary, recovery-focused team, working collaboratively with health professionals, carers, and families to provide integrated and person-centred care.
Responsibilities
- Undertake statutory social work duties in line with the Social Services and Well-being (Wales) Act 2014 and Mental Health Act 1983.
- Assess the needs and circumstances of individuals and families, developing tailored care and support plans.
- Act as Care Co-ordinator, ensuring care packages are well planned, monitored, and evaluated.
- Attend Mental Health Tribunals and contribute to care reviews and planning meetings.
- Carry out risk assessments and take appropriate steps to safeguard vulnerable individuals.
- Work collaboratively with internal and external agencies to promote recovery and maximise service user participation.
- Maintain accurate, timely, and confidential case records.
- Qualified Social Worker registered with Social Care Wales.
- Proven experience in mental health social work, preferably within a community or multi-agency setting.
- A valid driving licence and ability to travel as part of the role.
- Knowledge of WCCIS (Welsh Community Care Information System) is advantageous.
