Job Title: Social Worker – Community Mental Health Team
Location: Central Clinic, Orchard Street, Swansea
Contract: Fixed-term, initially 3 Months - Potential to be extended
Working Pattern: Hybrid (office-based, work from home, and client visits in hospitals and private homes)
Salary: £38.43 per hour via Umbrella, £30.50 PAYE

We are seeking a motivated and skilled Social Worker to join the City and County of Swansea's Community Mental Health Team. This role offers a dynamic mix of office, remote, and community-based work, providing essential support to adults experiencing mental health challenges.

Key Responsibilities:
- Conduct assessments and provide support for adults within the community and hospital settings.
- Prepare, write, and submit professional reports.
- Attend Mental Health Tribunals as required.
- Collaborate effectively with multi-disciplinary teams to ensure high-quality care.
- Visit clients in their homes and hospital settings to provide personalized support.
- Proven experience in mental health social work.
- Strong report writing and documentation skills.
- Experience attending Mental Health Tribunals.
- Ability to work both independently and as part of a team.
- Knowledge of WCCIS (Welsh Community Care Information System).
- Flexible working arrangements with a combination of home and office-based work.
