Learning Disabilities Social Worker - Middlesbrough Council
Service Care Solutions are recruiting for an experienced Learning Disabilities Social Worker to join Middlesbrough Council on an ongoing contract.
Rate of Pay:
The successful candidate will manage a varied caseload and lead complex social work interventions, working closely with a multidisciplinary team including psychiatrists, psychologists, nurses, and criminal justice professionals. You will undertake Care Act 2014 assessments, determine eligibility for services, develop strengths-based support plans, and complete Mental Capacity Act 2005 assessments and best interest decisions where required.
You will also play a key role in Section 117 aftercare planning, supporting the discharge of individuals from inpatient and secure settings back into the community. The role involves assessing and managing risk, responding to crisis situations using a trauma-informed approach, and providing specialist social work input into safeguarding and care planning processes to achieve positive outcomes for vulnerable adults.
