Social Worker - Home First Hospital Team
Location: Morriston Hospital, covering Singleton, Neath Port Talbot, and Gorseinon
Are you an experienced Social Worker seeking a rewarding opportunity within a dynamic hospital setting? We are currently recruiting 2 full-time Social Workers for the Home First Hospital Team on a temporary contract basis. This is an excellent chance to make a meaningful impact in patient care while working in a supportive and professional environment. The contract is initially set for 3 months with the possibility of extension, depending on the needs of the service.
Position Summary:
As a Social Worker in Home First Hospital Team, you will be responsible for providing vital social work support within hospital and community settings. You will work predominantly at Morriston Hospital, with potential coverage at Singleton, Neath Port Talbot, and Gorseinon. This role involves a blend of hospital-based and community work, requiring excellent communication skills, compassion, and a strong understanding of social services.

Candidate Responsibilities:
- Conduct comprehensive assessments of patients’ social needs
- Develop tailored care plans in collaboration with multidisciplinary teams
- Support patients in the hospital and community to ensure safe and appropriate care arrangements
- Facilitate discharge planning and liaise with relevant external agencies
- Maintain accurate records in line with organizational policies
- Provide advice and support to patients and their families
- Qualified Social Worker registered with Social Care Wales.
- Proven experience in mental health social work, preferably within a community or multi-agency setting.
- Strong understanding of relevant legislation, safeguarding responsibilities, and care coordination.
- Excellent written and verbal communication skills, with experience writing reports and presenting in formal settings.
- A valid driving licence and ability to travel as part of the role.
- Knowledge of WCCIS (Welsh Community Care Information System) is advantageous.
