Support Worker – Learning Disability Floating Support Norton Place, Westward Housing
£16.32 per hour umbrella
37.5 hours per week
Monday to Friday, 9:00am–5:00pm

We are recruiting a Support Worker to join a Learning Disability Floating Support Service in Norton Place. You will support adults to live as independently as possible, engage with their local community, pursue personal interests and develop practical life skills.
The role will involve managing a caseload and delivering person-centred support to customers in their own homes and within the community.

We are recruiting a Support Worker to join a Learning Disability Floating Support Service in Norton Place. You will support adults to live as independently as possible, engage with their local community, pursue personal interests and develop practical life skills.
The role will involve managing a caseload and delivering person-centred support to customers in their own homes and within the community.
Key responsibilities include:
- Creating, reviewing and delivering individual support and safety plans.
- Supporting customers with housing and tenancy matters, welfare benefits, budgeting, debt and accessing local services.
- Helping individuals develop independence, confidence, positive relationships and access to employment, training or education.
- Identifying and managing safeguarding concerns and risks in line with policy and procedure.
- Working closely with external agencies, healthcare professionals and other support services.
- Maintaining accurate case notes, risk assessments and support plans on internal systems.
- Supporting with group and one-to-one sessions, tailored to individual needs.
- Working independently, managing your diary and prioritising visits effectively.
- Experience supporting adults with learning disabilities, mental health needs, autism, substance misuse, homelessness or other complex support needs.
- A good understanding of safeguarding, professional boundaries, support planning and risk management.
- Experience in supported housing, social care, health or a similar support-focused setting.
- Strong communication, organisation and report-writing skills.
- Confidence using Microsoft Office and case management systems.
- An Enhanced Adult DBS on the update service and satisfactory references, ideally already in place.
