The Role As a Transitions Support Worker, you will:
- Build positive, empowering relationships with young people referred to the service.
- Carry out support needs assessments and create tailored plans to help young people achieve their personal goals.
- Support young people to access education, training, employment, positive activities, and other essential services.
- Deliver group sessions and positive community activities that build life skills, resilience, and independence.
- Liaise with partner agencies on behalf of young people and advocate for their wishes, needs, and rights.
- Ensure safeguarding responsibilities are met, following all statutory and departmental procedures.
- Maintain accurate, timely casework records using the service’s digital systems.
- Attend reviews, home visits, planning meetings, supervision, training, and service development activities.
- Promote equality, anti-discriminatory practice, and respect for cultural, religious, sexual, and individual needs.
- Support with practical duties such as preparing properties and completing Fire Safety checks.
About You Essential Qualifications
- NVQ Level 3 (Children & Young People / Adult Social Care) or equivalent.
- Experience working with Cared for Children, young people, and families.
- Strong understanding of child protection procedures, assessment, and planning.
- Ability to carry out direct work with children and young people in their homes and communities.
- Excellent communication, organisation, and IT skills.
- Ability to build trust, support emotional resilience, and work empathetically with those under stress.
- Knowledge of human development, attachment theory, and relevant legislation for care leavers.
- Commitment to anti-oppressive and anti-discriminatory practice.
- Ability to fulfil all spoken requirements of the role confidently in English.
- Experience supporting families with substance misuse concerns, domestic abuse, or complex needs.
- Understanding of care planning, court processes, and parenting capacity assessments.
- Experience analysing parent-child interactions.
- Knowledge of local networks and community resources.
- Full driving licence and vehicle for business use.
